The convenience of the SF-12v2™ with the detailed output of the SF-36v2™ is combined in this 8-question survey, the newest of the "Short Form" instruments. Each of the eight dimensions of health measured by the SF-36v2™ is represented by a single question. A health profile consisting of the 8-scales and two summary measures describing health-related quality of life is generated upon survey completion.

The eight items of SF-8™ also serve as the foundation of the Dynamic Health Survey™, representing the "tip of the iceberg" for the DynHA™ item pools on those eight dimensions.
