This 12-question survey generates an abbreviated health profile consisting of two summary measures describing health-related quality of life. It should take about 3 minutes to complete.

The SF-12v2™ has been shown to yield summary physical and mental health outcome scores that are interchangeable with those from the SF-36® in both general and specific populations. It has been used extensively as a screening tool. Because of its brevity, it is frequently embedded into longer, condition-specific surveys.
