Dynamic SF-36® Health Survey

The Dynamic SF-36® Health Survey is a tool used to measure health-related quality of life. It measures a person's ability to function on the job, at home and in social situations.

The survey was developed by an international team of clinicians and measurement experts, the same research developed the SF-36® Health Survey. The survey generates a health profile consisting of 8-scales and two summary measures for physical and emotional well-being.
