This privacy statement discloses the
privacy practices for the web site www.amIhealthy.com.
QualityMetric Incorporated/www.amIhealthy.com
is a licensee of the TRUSTe Privacy Program. TRUSTe is an independent,
non-profit organization whose mission is to build users' trust and confidence
in the Internet by promoting the use of fair information practices. Because this web
site wants to demonstrate its commitment to your privacy, it

Information Collection and Use
QualityMetric Incorporated is the sole owner of the information collected on
this site. We do not sell, share, or rent this information to others in ways
different from what is disclosed in this statement. We do not sell any products or services
through this web site or accept any advertising.
Though we make every effort to preserve user privacy, we may need
to disclose personal information when required by law wherein we have a good-faith belief that
such action is necessary to comply with an appropriate law enforcement investigation,
current judicial proceeding, a court order or legal process served on our Web site.
Registration
After registering, a user has the option to skip
any questions in the survey. If you complete a question and proceed to the next
page you cannot go back to the previous questions and change your answer. You
can register before you take a survey. Once you are registered, the
results of your surveys are saved and you can track the changes to your scores
over time. You or your sponsoring organization only needs to register you once.
During registration a user gives us their contact
information (such as name and email address). This information is used to contact
the user about services on our site for which they have expressed interest. It is
optional for the user to provide certain demographic information, but we encourage
you to answer all requests so that we can provide you with a more personalized and meaningful
survey report. It is especially important for you to supply your age and gender so
we can tell you how your survey scores compare to your peers, those with the same
age and gender as you. After you take a survey on our site, you decide whether to
print a paper copy of your survey results. You also decide if you want to share the survey
results with your health care provider or anyone else. No one other than you can
view or print your specific survey results.
When you register on our site, we assign
you a unique identification number. This is how you are identified as you use
the site. This ID number is not linked to any other web site's identification
numbers.
Group ID Number
Some of our users are enrolled in
amIhealthy.com by their doctor's office. These users have given their consent
to their doctor to participate in the program. Their doctor purchased a block
of surveys to be administered to their patients on the amIhealthy.com web site.
Patients of the doctor are issued a group ID number prior to their registration.
The group ID number allows us to identify all the users who are part of the
doctor's group. Your doctor can then view the entire group's survey results in
an aggregate manner or can request individual survey results. If you do not want
your doctor to be able to review your individual survey results, do not supply
the group ID number during your registration. You should also speak with your
doctor directly if you have concerns about participating in their program.
Background Information Survey
A user may choose to complete a Background
Information survey. The information collected from this survey is aggregated and
used internally by QualityMetric Incorporated to improve existing surveys and scoring
methods.
Cookies
A cookie is a piece of data stored on the consumer's hard drive, which
contains information about how the consumer used the web site. Cookies enable us
to track and target the interests of our consumers to enhance the experience on
our site. Usage of a cookie is linked to personally identifiable information while
on our site. By setting a cookie on our site, the user would not have to log in a
password more than once, thereby saving time while on our site. Cookies are also
used to recognize survey respondents and to link and maintain survey information
like scores. If a user rejects the cookie, they may still use our site. The only
drawback is that the user will be limited in some areas of our site. We also use
cookies to prevent users under 18 years of age from gaining access to our surveys.
Log Files
We use Nettracker software which uses IP addresses, browser type, ISP,
referring/exit pages, platform type, date/time stamp and number of clicks to
analyze trends, administer the site, track user's movement and gather broad
demographic information for aggregate use. IP addresses, etc are not linked to
personally identifiable information. We do not sell or share your IP addresses,
etc.
Email
At times we may
request your email address and ask permission to contact you. This is optional
and it is up to you to decide whether you want to receive this service. You must
"opt in" and supply us with your email address. If your doctor or sponsoring
organization has requested it, you may receive reminders when it is time to take
another survey. Also if your doctor or sponsoring organization has requested it,
you might also receive an email with follow-up questions about our health or our
survey experience. Selection for these follow-up questions will either be at
random or may depend on your previous survey score. We may also contact you if
you have forgotten your password and requested that we email it to you. You can
discontinue any of these services whenever you like by contacting
us at webmaster@amihealthy.com
or when you receive an email, key "unsubscribe" in the subject line of your
response. We do not sell or share your email address.

This web site contains links to other sites. Please be aware that we,
QualityMetric Incorporated, are not responsible for the privacy practices or content of
such other sites. We encourage our users to be aware when they leave our site
and to carefully read the privacy statements of each and every web site that
collects personally identifiable information. This web site's privacy

Sharing
We share aggregated demographic information or survey scores and item responses
with our business partners. This is not linked to any personal information that
can identify or be linked back to an individual person.
Security
We take every precaution to protect the information you have shared with us.
When users submit sensitive information via our website, your information is
protected both online and off-line. When our registration form asks you to enter
sensitive information (such as date of birth), that information is encrypted and
is protected with the best encryption software in the industry - SSL. While on a
secure page, the lock icon on the bottom of Web browsers such as Netscape
Navigator and Microsoft Internet Explorer becomes locked, as opposed to
un-locked, or open, when you are just 'surfing'. While we use SSL encryption to
protect sensitive information online, we also do everything in our power to
protect user-information off-line. All of our users' information, not just the
sensitive information mentioned above, is restricted in our offices. None of our
employees require consumer specific information to perform their jobs. They only
deal in the aggregate or grouped data without personally identifiable
information.
For extra security our employees use
password-protected screen-savers when they leave their desktop computer. ALL
employees are kept up-to-date on our security and privacy practices. Annually,
as well as any time new policies are added, our employees are notified and/or
reminded about the importance we place on privacy, and what they can do to
ensure our customers' information is protected.
Finally, the servers that we store
personally identifiable information on are maintained and kept in a locked,

Correction/Updating Information:
If a user's personally identifiable information changes (such as your name or
zip code), or if a user no longer desires our services, we will correct, update
and/ or remove that user's personal data at their request.

Notification of Changes
If we are going to use your personally identifiable information in a manner
different from that stated at the time of collection, we will notify you by
email. You will have a choice as to whether we use your information in this
different manner. However, if you have opted out of communication with us, then
you will not be contacted and your personal information will not be used in this
manner. Also if we make any material changes in our privacy practices that do
not affect user information already stored in our database, we will post a
prominent notice on our web site notifying users of the change. In some cases
when we post the notice we will also email users, who have opted to receive
communication from us, notifying them of the changes to our privacy practices.
